Capital Counties Unveils 2025-2027 Community Health Improvement Plan

Strategic Health Plan Launched to Enhance Well-being in Capital Counties

The Healthy! Capital Counties initiative has unveiled its 2025–2027 Community Health Improvement Plan (CHIP), aiming to boost health and wellness across Clinton, Eaton, and Ingham counties. This strategic endeavor transforms insights from the 2024 Community Health Assessment (CHA) into actionable, collective measures for the forthcoming three years.

As the second collaborative CHIP for the region, this plan is a result of the ongoing alliance between the Barry-Eaton District Health Department, Ingham County Health Department, and Mid-Michigan District Health Department. Focused on health equity, policy, social well-being, and sustainable community engagement, the 2025 CHIP sets a comprehensive agenda.

The plan targets three main concerns identified in the 2024 CHA: Access to Care, Behavioral Health, and Housing. To tackle these issues, 23 community partners have pledged to carry out 73 activities throughout the Tri-County area.

CHIP serves as a unified blueprint guiding health departments, partners, and community members towards a healthier and more equitable future. Healthy! Capital Counties urges all involved to maintain their commitment and support this collective mission to enhance community health.
